| About | Jack fell in front yard in September 2019; fractured ball in hip and had a stroke during hip replacement surgery making him immobile. He was in and out of 4 assisted living over last 4 years and finally moved to Lady Lake Senior Living in Lady Lake, FL where he was admitted to Memory Care from where he passed. |
