A Construction Worker for A&D Remodeling, he was a member of Corpus Christi Parish in McKeesport. In addition to his mother, he is survived by his wife, Debra M. Powell Formo; daughter, Ashley Lynn Formo of Pittsburgh; stepdaughter, Crystal G. Winterhalter of Pittsburgh; stepson, James D. Winters of North Huntingdon; sister, Carol Anne (Robert T., Sr.) Masters of North Huntingdon; five granddaughters; half brother, Michael R. Formo of Fergus Falls, MN; a niece and nephews.
